During national Integrated Care in Action week, Devon’s Integrated Care Programme outlines some of the key projects it is leading.

Across Devon, plans are being developed that combine local priorities with the national commitments of the NHS Long Term Plan. These include:

• Supporting Devon’s Population Health Management programme to help people with complex needs stay healthy
• Joining up support between Primary Care Networks, voluntary groups and mental health teams to care for people at home and in their communities
• Working with our social care independent sector to better coordinate care and reablement.

Integrated care joins up acute, community, primary, mental health and social care services to improve the health and wellbeing of local people.
