Five Primary Care Networks in Devon are taking part in a innovative scheme to improve health and wellbeing in their area using population health data.
Primary Care Networks are groups of GP practices who work together to improve care for their patients. Working with a range of organisations – including Public Health, NHS Devon CCG, NHS acute and mental health providers, the South West Academic Health Science Network, PENCHORD and local communities – the networks will use population health management techniques to improve people’s health and wellbeing.
Five Primary Care Networks across Devon were identified to work on a 20-week intensive NHS England programme starting at the end of March:
- Barnstaple
- Pathfields (Plymouth)
- Woodbury, Exmouth, Budleigh
- West Devon
- West Newton
